Membership Transfer/Freeze/Cancellation

Please fill out this document if you have a family or monthly membership and want to have it frozen, would like to switch to another membership, add or drop a person from your family membership, or want to cancel your membership. All requests must be made 48 hours prior to the billing date, which is the first day of each month.
