CLIMB TOOWOOMBA – TEMPORARY AUTHORITY WAIVER FORM (SCHOOL GROUP)

PURPOSE:
This form grants temporary authority for the above-named teacher to take full responsibility for the student listed when a completed parent/guardian safety waiver has not been submitted to Climb Toowoomba prior to participation.

DECLARATION:
I, the undersigned, confirm that the parent/guardian of the student listed above has not yet completed Climb Toowoomba’s mandatory Safety Waiver.
But has completed a school excursion form and is fully aware of today's activity.
In lieu of a signed parent/guardian waiver, I hereby:

*Accept full responsibility for the student’s safety and conduct while participating in activities at Climb Toowoomba on today’s date.

*Acknowledge that indoor rock climbing involves inherent risks, and I voluntarily assume responsibility for these risks on behalf of the student.

*Agree to closely supervise the student at all times during the session and ensure they follow all gym safety rules, instructions, and protocols.

*Commit to contacting the student’s parent/guardian to have a completed and signed Safety Waiver returned to Climb Toowoomba within 7 days of the date above.

ACKNOWLEDGEMENT:
I understand that this form is a temporary measure to ensure student participation in today’s activity and that continued participation without a signed parental waiver is not permitted.
I also understand that Climb Toowoomba reserves the right to refuse participation at any time if safety obligations are not met
