Unsupervised Rope Climbing Registration Form

This form and associated assessments evidence that the climber is significantly known to the Indy Climbing Wall as a competent top rope, lead climber and wall user and a parent or legal guardian has given informed consent for the climber to climb unsupervised and fully understands the risks involved.
NOTE: For the purpose of this form ‘Supervisor’ means the parent or legal guardian who has given informed consent. The ‘Supervisee’ is the climber.
