2 - New Adult Climber (Supervised)

Guidance

• This form is to be filled out by Novice Climbers aged 18yrs or older.
• This form is valid on the day of completion only; another supervised climbing form must be completed for each subsequent visit as a Novice Climber.
• The Supervising Adult must be fully registered at the centre and have completed the Supervisor Proficiency Test. Please ask a member of staff if you are unsure whether the Supervisor had completed the proficiency test.
• Both Novice Climbers and the Supervisor must have read the Participation Statement, Conditions of Use and Code of Practice.

Participation Statement:

“All climbing activities have a risk of serious injury. Participants must recognise that even if they follow all good practice there may still be the risk of accident and injury. It is the responsibility of the participant to adhere to the conditions of use.”
