• If you are under 18 years of age DO NOT complete this form, you will need to come to the centre with a parent or guardian to complete the correct form.
• If you are not a competent climber then please DO NOT complete this form.
• The completion of this form not only allows you access to climb unsupervised but also allows you to supervise Novice Climbers and U18's within the centre.

Participation Statement:
