4 - Family Registration Form

Guidance

This form is for an Adult to register themselves as well as a maximum of 5 Climbers under the age of 18.
U18's must be 4yrs or older.
If you are not a competent climber then please DO NOT complete this form.
Climbers aged between 4yrs and 17yrs must be properly supervised at all times within the Centre.
Climbers aged 14yrs+ can complete a Junior Sign Off to be able to climb unsupervised within the Centre. 7
This form allows the adult to supervise other climbers within the centre.

Participation Statement:

“All climbing activities have a risk of serious injury. Participants must recognise that even if they follow all good practice there may still be the risk of accident and injury. It is the responsibility of the participant to adhere to the conditions of use.”
