OLD DELETE - Supervisee Registration

Guidance

This form should only be filled out by registered customers aged 18 or over wishing to link additional supervisees to their account. Prior to climbing and supervising you must have filled the relevant forms and have the associated proficiencies in place. If this is not the case then please stop filling the form and speak to a member of staff.

This form allows you to register and link up to 5 participants to your account. Please be aware that you must only attend and supervise a maximum of two participants on any single visit. We can NOT allow participants to switch in and out with one another by waiting in the cafe. Please discuss this with a member of staff for further explanation.
