Novice Supervision Form

This form must be completed by BOTH the Novice Climber and the Supervising Climber.

Only a registered roped climber may supervise novices and only two novices at any one time. If you are new to the centre and wish to supervise a novice, please fill out your own Acknowledgement of Risk form before completing the Novice Supervision form.

If the novice climber is under the age of 12 or not able to understand/sign the form, a responsible adult must complete the form on their behalf.

You must complete one form for each novice/supervisor combination.

Once you have submitted the form you must visit reception to have it reviewed and processed.
